Central Coast Renal Care providers will be joining Dignity Health as a Pacific Central Coast Health Center in 2024!
Our New Locations
Beginning February 2024 Central Coast Renal Care providers will begin operating out of two Dignity Health Pacific Central Coast Health Centers locations - Arroyo Grande and Templeton.
Dr Michael DiBiase, along with Diane De Vos-Schmidt, FNP, will be based our of the PHC Specialty Center in Templeton at 350 Posada Lane, Ste 202, Templeton, CA 93465. Their office can be reached via phone at 805-434-5497.
Dr Brian Penton will be based out of the PHC Fair Oaks Specialty Center located at 850 Fair Oaks Ave, Ste 220, Arroyo Grande, CA 93420. His office can be reached at 805-547-2224.
Medical Records Requests & Care Transfers
Established patients looking to Transfer their Care to one of the new Dignity Health PHC locations listed above will need to complete the Dignity Transfer of Care form linked below. Patients wishing to have their records sent to an alternate provider will need to complete the Generic Medical Records Request form.
